Care Direct (Salford) offers a full and comprehensive home assessment of you or your family members needs before the service is implemented. It may be that a family member, friend or neighbour may wish to be present on our first visit. Generally this helps to get as much information as possible in providing the appropriate support for you or your family members needs.
Support can range from:
• Support with personal care such as hair dressing appointments or bathing support
• Support in the community ( shopping, socialising, visiting garden centres ) for example
• Medication support including visiting the
local chemist
• Emotional support at such difficult times
• In-house domestic tasks such as laundry support and meal time preparation
• Benefits and budgeting including support collecting your pension or benefits with yourself
• Attending Dentist, Doctors or hospital appointments that are important but difficult to attend for whatever reason
• Support with maintaining your religious following.
• Support with applying for voluntary work or access to employments schemes
• Supporting a Carer to access appropriate support for their own needs
• Building confidence to be at home following a hospital admission
Care Direct (Salford) will support you or your family member directly from hospital whether it’s the local general or psychiatric hospital as part of the essential discharge aftercare in meeting your current needs and supporting you to return back home.
Clients will receive
a
booklet that explains the service philosophy and complaints procedure including useful contact telephone numbers once the support is delivered
Care Direct (Salford ) provides support for clients who suffer from:
• Any dementia including early onset dementia; for example Alzheimer’s Disease, multi-infarct dementia or Korsakoffs Syndrome
• Schizophrenia and psychosis
• Depression
• Learning Difficulties
• Obsessive Compulsive Disorder
• Bi-polar Disorder
• Sensory impairments
• Challenging behaviours
• Grief and bereavement issues
• Or other vulnerable individuals
